• Individual Community Support Services
  One-on-one assistance to help individuals build daily living skills, navigate the community, and increase independence.
  
• Community Living and Supports (Individual)
  Personalized support for individuals in their home or community to assist with daily activities, transportation, and personal care.
   
• Community Living and Supports (Group)
  Group-based services that promote social interaction, communication, and independent living skills in a community setting.
   
• Community Living and Supports for Children (Group)
  Group services tailored for youth, focusing on age-appropriate development, peer interaction, and community engagement.
   
• In-Home Case Management
  Coordination of care and linkage to services that support individuals and families in the home and community environment.
   
• Group Supported Employment Maintenance (IDD)
  Ongoing job coaching and support for individuals with intellectual and developmental disabilities in a group employment setting.
   
• Home Care Services for Adults
  In-home support to assist with activities of daily living such as bathing, dressing, mobility, and light housekeeping.
   
• Home Care Services for Children
  In-home care and supervision focused on the unique needs of children, including support with routines and safety.

 Raising A Standard, Inc. provides 1915(i) services under Trillium Health Resources, Partners Health Management, and Vaya Health to support individuals with behavioral health needs and intellectual/developmental disabilities. These services are person-centered and designed to promote independence through community integration, daily living support, and skill development. Our goal is to help each participant thrive in the least restrictive environment while maintaining dignity and choice. 

 Raising A Standard, Inc. is an approved Medicaid provider offering behavioral health and rehabilitation services. We also accept private insurance and cash payments to ensure flexible access to care for all individuals and families.
